The Climax and Resolution

In the final act of Shutter Island, Teddy discovers that he is not a U.S. Marshal, but rather an inmate named Andrew Laeddis. 

In the scene, Andrew and Dr. Sheehan sat together on some steps to smoke. Andrew was just told his entire identity as US Marshal Edward “Teddy” Daniels was made up by him to escape from the reality that he killed his wife. Andrew, in short, has realized who he is. 

Additionally, Dr. John Cawley revealed to Andrew that he, at one point, found out who he was before but he then relapsed back into forgetting himself, assuming the identity of Teddy, and beginning the investigation all over again. Cawley informs Andrew that he will be lobotomized if his recent treatment fails. From afar, Dr. Cawley observes them.

As Andrew and Sheehan continue to talk, Sheehan realizes that Andrew seems to have relapsed back into his identity as Teddy. Andrew continued to talk as if he were an investigator investigating the mystery of Shutter Island. 

When he realizes it, he looks over to Dr. Cawley and shakes his head, giving Cawley a hint that the treatment hasn’t worked. So Cawley goes in to bring the men that will lobotomize Andrew. Before those men arrive, Andrew makes a statement “Is it worse to live as a monster, or to die as a good man?
